Insurance

Please bring your insurance information with you during your first visit and at any time there is a change in your policy.

Please note that Health Insurance and other benefits may cover some or most of the costs of your treatment(s) but most require that you share in the financial responsibility. You may be required to pay a co-payment, deducible, co-insurance, or other expenses. Please be prepared to pay your financial responsibility at the time services are rendered.

Please note that it is your responsibility to obtain referrals from your primary care physician if they are required, and to keep us continually updated with your most current insurance information as well as your personal address and contact information as changes occur.

We have contractual agreements with many of the private insurance plans in our area. We however cannot guarantee participation with every insurance company, plan, or insurance product and because of the complexities involved we encourage you to verify our participation by contacting your insurer directly. We gladly file claims on your behalf for the medical services we provide.

Laboratory, Hospital and Precertification Restrictions and/or Requirements

Please check with your insurance company and make us aware of plan restrictions requiring you to utilize a specific Clinical Laboratory and/or Hospital. If we recommend surgery for you, we also recommend you contact your insurance company to ask if they require precertification or preauthorization prior to surgery.
